Please read the following before booking:

• If you have not seen Dr. White for 12 months or more be sure to book a Progress Exam

• If you are a current patient of Dr. White but have a new injury, new symptom or concern please also book a Progress Exam

• If you are being seen for a Workplace injury please select the worker’s treatments that applies and inform your practitioner upon arrival

• Please be advised of the 24 hour cancellation policy that is in place for all practitioners. If you cancel or reschedule within the 24 hour time frame there is a $50 for Initial Chiropractic Assessments, $20 fee for Chiropractic subsequent visits, and $50 for both Massage Therapy and Physiotherapy appointments. By booking online you are agreeing to this policy and fees will be processed using the payment method on your file.
